To complete the registration process, parents/guardians are required to submit proof of vaccination record. 

California schools are required to verify immunization records for all new students and all students entering the 7th grade. 

Students may not be able to begin classes until the students’ complete immunization record has been received and verified by the health office.

 

New students and incoming 7th graders MUST have the following immunizations:

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
    (4 doses needed if one was given on or after 4th birthday;  3 doses needed if one was given on or after 7th birthday)

  • Tdap (at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday) New requirements for incoming 7th graders!

  • Polio (OPV or IPV) — 4 doses
    (3 doses OK if one was given on or after 4th birthday) 

  • Hepatitis B — 3 doses

  • Measles, Mumps, and Rubella (MMR) — 2 doses (both must be given on or after 1st birthday)

  • Varicella (Chickenpox) — 2 doses New requirement for incoming 7th graders!

Any new student enrolling from outside Orange County is required to complete a  TB assessment.  TB assessments can be done by scheduling a consultation with a District Nurse.  Please call Health Services at 714-999-0814 to make an appointment.   

Students coming from another country are required to obtain a TB test.  To get a TB test contact your doctor, clinic, or health department.  

Submit your negative or positive results to the Brookhurst health office.

About the HEALTH OFFICE
 
Brookhurst Junior High School is staffed with a full time Health Technician, trained in first aid, AED and CPR. The primary role of the Health Technician is to provide basic first aid, administer medication when ordered by a physician, to assist students with other various health concerns and to be a resource for parents to access health related resources.  The school health office maintains and oversees health and immunization records on all students.  An assigned District Nurse also oversees special needs at several school sites, including Brookhurst Jr. High, and is available throughout the day as needed.  Anaheim Union High School District also has a Health Service Center located at the District Office.  
 
 
The Parent's Role 
Parents are asked, particularly at this time, to assess their student's health each morning before sending them to school.  Parents should always notify the attendance office if a student is being kept home in order to clear the absence (see the attendance page for more information).  Once a student has been sent to school, it is the discretion of the health office to determine if a student should remain at school or be sent home.     

Children are in close contact with one another at school. If parents keep their children home at the first signs of illness, it will help us prevent the spread of that illness to other children in the classroom. The following are some guidelines to help you decide when to keep your ill child home from school. These are the guidelines and symptoms we look for at school to determine when to send children home from school.

 · Nausea, vomiting, or diarrhea

 · Earache, ear drainage

 · Sore throat

 · Persistent cough

 · Runny nose if the discharge is yellow or green

 · Oral temperature over 100 degrees. Temperature should remain normal for 24 hours (without fever reducing medication)  before the student returns to school.

 · Headache or stomach ache in combination with other symptoms (i.e., cough or sore throat)

 · Unidentified rash or communicable skin rash. Child may return to school following proper treatment or with a doctor's note when requested.

· Pinkeye (conjunctivitis) in one or both eyes. The whites of the eyes will appear pink or red, and feel painful and itchy. There may be yellow, green or white matter in or around the eye or lashes. 

Health Office Visits

Except in the case of an emergency, all students must have a pass from their teacher to visit the health office.  Walk-in's during passing period will be returned to class for a pass. For safety reasons, students are not allowed to contact a parent with their cell phone for either emergency or non-emergency health concerns.   Parents are encouraged to put the school phone number in their cell phones so they can identify a call from the school easily and quickly.  The health office will contact a parent/guardian first if it is determined a student needs to go home.   No student will be allowed to leave with another adult (even a relative) unless that person is listed as an emergency contact.  It is the responsibility of the parent/guardian to keep their emergency contact information current, which can be done by logging onto the Aeries Parent Portal.   If a student should contact a parent directly regarding a health concern, parents should contact the health office FIRST before coming to the school to pick up a student. 
 
Medications
All medications which are to be administered at school should be taken to the health office (along with the necessary medication form completed by a physician) by a parent.  The only medication a student may carry themselves are an inhaler or an epi-pen and then only if authorized by a physician by submitting the medication form completed by a physician to the health office first.   Any other medications found on a student, including over-the-counter medication, will be confiscated and a parent will be required to pick up the medication.  If you have any questions or concerns, you can reach Ms. Blumberg by phone or email.  By law, the school cannot administer any medication, including over the counter medication, or treatment without parent permission and a physician's order. We can assist you with this process as well as provide the necessary forms by contacting the health office.  (Note: a separate form is required for each medication to be either carried by the student and/or stored in the health office)

IMPORTANT HEALTH CONDITIONS
Please let the Health Office know if your child has a Life Threatening illness such as diabetes, seizure disorders, or severe allergies (Bees/Food/Medication). We want to be prepared to provide safe care for your child.  During the registration process, you will be given the opportunity to disclose medical conditions of all types.   In the case of a life-threatening or special medical concerns, it is recommended you contact the health office so we can notify teachers when appropriate, provide necessary forms when needed, and to be prepared in case an emergency occurs.

Students who do not have a currently required vaccine due to a medical condition or history of disease (such as chickenpox) must submit a medical exemption through the California Immunization Registry - Medical Exemption division (also known as CAIR-ME).   Directions are found below.
